This website is used to provide information about my services. I have a “Resources” and “Contact” page that request personal information if you wish to engage in my services. If this information identifies you, your information is kept private by me and used only for the services you have engaged in. By visiting my site, your digital data is also made available to me and my website management platform. 

Information that is gathered from visitors:

When you engage with a service request on my site, the following information might be gathered: your name, contact details and service requests.

By visiting my site, my analytics automatically collect certain information about your device as well. This might include information about your IP address, web browser, social media platform and time zone. Some of the cookies on your device might also be recognized. When you view my site, information about the pages and products you look at is stored. The websites or search terms used by you to bring you to my site are also stored. 

The information mentioned above is collected on this site and stored as “cookies,” which becomes analytical information about my visitor preferences. This information is typically anonymous but can also carry some unique data that could be traced to your device. Your actions on my site are logged and tracked to support service provision. Data on how you browse this site might also be stored to support service provision. 

Personal information used: 

When you enter your private information on the “Resources” or “Contact” pages of this site, your personal information is stored. This information is only used by me to make contact with you, for a particular service requested by you on the order form. Engaging in this platform - for yourself, your child or someone else - is the provision of your consent to use this information in this manner.

I collect and store the information you provide in order for me to adhere to administrative requirements of the practice only, and to provide services requested by you. I will use your personal information only for the purposes for which it was collected.

Your personal data may be stored by my service provider in accordance with this policy and privacy laws, to the best of my knowledge. 

Data retention:

I collect and store information about you and your child in order for me to continually assess, treat and manage your clinical best interests. Client records are kept in secure physical/electronic form for a period as regulated by professional standards set out by the HPCSA, and will be safely disposed of/destroyed thereafter. This refers to 6 years for adults, until a 21st birthday for children and for a lifetime for those with an intellectual disability. 

Security measures:

I will, on an on-going basis, continue to review my security controls and related processes to ensure that your personal information remains secure. You are encouraged to adjust your own privacy settings to review your browser security as well, if desired. 

Rights: 

You have the right to request that I update, correct, or delete your personal information using the relevant forms as set out in Protection of Access to Information Act (POPIA) (Form 1 and Form 2). These can be requested directly from me or found online.

As per the PAIA and the processes outlined in the PAIA manual for this practice, you have the right to request a copy of the personal information that I hold about you, the copying and provision of which may be subject to payment of a legally allowable fee. The PAIA manual and Form C for requesting information can be requested directly from me or found online.
